Home Care packages are now "Support at Home"
From 1 November 2025, the Australian Government is introduced a new funding program called Support at Home, which will replace the current Home Care Package program.
At Grata Care Services, we’re here to support you through these changes every step of the way. We’ll help you understand what’s changing, and make sure you continue receiving the right support to live well at home and in your community.
What is Support at Home funding?
Support at Home offers a high level of specialised care and support services to our senior citizens aged over 65 years to enable them to safely remain in their own homes.
The new program offers a clear list of services grouped into 3 categories of; Clinical Supports, Independence and Everyday Living.
1. Clinical supports for more complex conditions requiring professional care
- Nursing
- Allied health and other therapeutic services
2. Independence services for support with daily tasks and mobility
- Personal care
- Social support and community engagement
- Therapeutic services for independent living
- Respite
- Transport
- Assistive technology and home modifications
3. Everyday living services to keep your home safe and functional
- Domestic assistance
- Home maintenance and repairs
- Meal preparation and delivery
What services can I receive?
The types of services will vary depending on the level of Support at Home you receive.
Once you have been approved for Support at Home, Grata can ensure you have access to the most useful services. We will work with you so that you make the most of your entitlements.
Services for wellbeing and independence
These services include personal care, nursing, allied health and therapy services, food and nutrition, specialised support.
Services for in-home safety
In-home care encompasses more than you might think. From cleaning and home maintenance, to minor home modifications, goods, equipment, and assistive technology.
Services to maintain community connection
Access transport services, social support services, or participate in community or club activities.

How do I receive Support at Home ?
The number of Government-funded Support at Home is limited, therefore it is important to be assessed in order to determine your eligibility.
The first step is to apply for an assessment online, or call My Aged Care on 1800 200 422. The process is quick and easy ‘make a referral“.
An assessor will determine the level of care to best suit your needs.
Once you have been approved, contact us! We are the service providers who work alongside you, to design the most effective care plan within your approved level of support.
Important update regarding Grata Care Services
On September 2nd, 2025, Grata Care Serviceswas acquired by Australian Unity. The acquisition will enable Grata Care Services to support the home health care needs of agreater number of Australians, allowing them to remain in their own homes for longer.
