Aged Care Assessments
Grata have been appointed by the Australian government to conduct assessments for people needing access to government funded care services, to accurately and comprehensively determine their required level of care and funding. Using the Department of Health and Aged Care’s Integrated Assessment Tool (IAT), our service is a vital step in connecting Australians with much needed residential aged care or home care services and support.
The New System
The 2018 Royal Commission into Aged Care Quality and Safety identified that the then-current system for aged care assessments was not meeting the needs of our community. Recommendation 28 of the commission proposed
The Australian Government establish a single aged care workforce empowered and trained to do all the assessments that they need across both home and residential care. This will ensure older people have a more streamlined assessment experience and receive greater clarity about likely services
Now in 2024 the Department of Health and Aged Care has implemented the Single Assessment System and appointed a new workforce to service the needs of our older Australians, of which Grata is a proud participant.
The Single Assessment System aims to simplify and streamline the assessment process while enhancing the experience for older people. It offers flexibility to improve access to assessment services if and when an individual’s needs change
The Integrated Assessment Tool is designed to evaluate and support individuals, with a focus on promoting their independence and achieving personal goals. We partner with assessment recipients to co-design their wellness, reablement & support journey.
The tool integrates various aspects such as health, social and personal needs to create a comprehensive and personalised plan for their ongoing care needs.
How It Works

If your application is successful, you will be referred for an assessment by an assessment provider, like us

An assessor will be scheduled to conduct your assessment at your own home or residence
The assessment will comprehensively review your needs and provide accurate information to determine if government-funded aged care services are suitable for you.

If you have requested home care services, the government will notify you of the outcome of your assessment and if you are eligible for funding, will provide you with a support plan
Your support plan will include information and resources as to how you can arrange the services and support you require. If you have been assessed for residential aged care funding, the government will liaise with the facility directly.
